Job summary

Catholic Charities Senior Communities in Baltimore is seeking a Service Coordinator to support residents’ stability, independence, and aging in place within an affordable housing setting. The role links residents with management, service providers, and partners while ensuring accurate records and HUD compliance.

Responsibilities include developing personalized plans, coordinating services, and fostering engagement across three locations, with a Monday–Friday schedule and flexibility to work

Qualifications

• Bachelor’s degree in social work, human services, gerontology, psychology, sociology, or a related field.
• Two years’ experience providing social services or service coordination, including assessing needs and developing service plans.
• Knowledge of local services for the elderly and people with disabilities.
• Equivalent combination of education, training, and experience may be considered.

Responsibilities

• Develop personalized service plans and connect residents to resources to support well-being.
• Build collaborations with community partners to expand access to services and improve outcomes.
• Promote resident engagement, social connection, and self-advocacy.
• Respond promptly to resident needs and safety concerns with appropriate supports.
• Coordinate with families and staff to ensure continuity of care and accurate documentation.
• Connect residents with essential resources to support independence and decision-making.
• Support a strong volunteer program and promote resident safety and independence.
• Complete required training and apply HUD guidelines to support residents.
