Job summary

The Center for Human Services is seeking a Service Coordinator to manage a diverse caseload, build community supports, and advocate for individuals with developmental disabilities. You will collaborate with families, care teams, and partner agencies to develop and implement person-centered service plans.

Responsibilities include identifying resources, coordinating services, monitoring quality, and maintaining timely documentation.

Qualifications

• Bachelor’s degree or RN license required.
• Reliable transportation and driving record required.
• Own vehicle for transportation to individuals’ homes for in-person visits.

Responsibilities

• Manage a caseload of varying ages and needs to improve independence and inclusion.
• Develop action plans and coordinate services with the individual, family, and care team.
• Identify community resources and link them to the individual’s plan.
• Monitor service quality and timely delivery; report any abuse or neglect as required.
• Document activities within three business days of TCM activity with accuracy and completeness.
• Participate in ongoing training and knowledge enrichment; contribute to team unity.
