State of Missouri is seeking a dedicated HCBS Coordinator to support seniors and adults with disabilities. You will conduct assessments, coordinate HCBS services, develop care plans, and travel within a multi-county region to visit participants and providers.

Qualifications

• Excellent communication and interviewing skills are required.
• Ability to work independently within a team and take initiative.
• Experience with elder or disability services is preferred.

Responsabilités

• Conduct initial assessments and reassessments to support least restrictive environments.
• Coordinate HCBS services by contacting participants and providers for service authorizations.
• Develop and implement care plans addressing unmet needs and provider choices.
• Travel locally to conduct home visits, hearings, and trainings; minimal overnight travel.
• Ensure program integrity, operations, and success; represent DSDS in administrative hearings.
