Job summary

Centene is seeking a Home Health care management RN for a hybrid role serving Hidalgo County, TX. You will assess complex/high-acuity members, coordinate services, and develop personalized care plans to promote quality and cost-effective care.

Responsibilities include collaborating with providers, updating care plans, educating members and families, and ensuring compliance with state/federal guidelines. Travel is required for visits; schedule is Mon-Fri.

Qualifications

• Requires graduation from an accredited school of nursing and 4–6 years of related experience.
• Active RN licensure in applicable state(s) and compliance with licensure requirements.

Responsibilities

• Evaluates care needs of highly complex/high-risk members and plans the best outcomes.
• Develops long-term care/service plans and coordinates with providers and resources.
• Collaborates with care teams to ensure person-centered care and services.
• Monitors progress and revises plans to meet member goals and safety.
• Educates members and families on disease processes, care options, and benefits.
