Job summary

Centene Corporation is seeking a hybrid RN Care Manager to support Duals AIP adult members with complex medical needs in Hidalgo County, TX. The role involves home visits, coordinating LTSS needs, and self-scheduling from a home office.

Schedule is Monday–Friday, 8:00 AM–5:00 PM CST with no on-call duties, evenings, weekends, or holidays. Requires nursing licensure and BSN preferred, with 4–6 years of related clinical experience.

Qualifications

• Graduate from an Accredited School of Nursing with 4–6 years of related clinical nursing experience.
• BSN preferred.
• RN licensure in good standing.

Responsibilities

• Evaluate the service needs of the most complex or high risk/high acuity members and develop a plan for the best outcome.
• Coordinate and manage between member/family and care provider team to ensure appropriate person-centered care.
• Monitor and revise care plans as needed to meet member goals and needs.
• Educate long-term care members and families about disease processes, care options, and benefits.
