Qualifications

• Graduate from an Accredited School of Nursing and 4–6 years of related clinical nursing experience.
• Bachelor's degree in nursing (BSN) preferred.
• 7+ years of clinical RN Case Management or Care Coordination in adult populations preferred.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome.
• Develops and continuously assesses long-term care plans / service plans with providers and resources.
• Coordinates between member/family and care team to ensure appropriate person-centered care.
• Monitors plans, conditions, and progress; revises plans as needed to meet goals.
• Educates members and families on disease processes, care options, and benefits.
• Acts as liaison between member, physician, and facilities; facilitates services and treatments.
