Job summary

Centene Corporation is hiring for a hybrid RN Care Coordinator role serving Brownsville, Harlingen, and San Benito, TX. The position combines home visits with remote work, requiring strong care coordination for adult members in home health, hospice, long-term care or managed care.

Schedule is Monday–Friday, 8:00 AM–5:00 PM CST with no on-call duties. Requires RN licensure and nursing degree, with travel to member sites as needed.

Qualifications

• Requires Graduate from an Accredited School of Nursing or a Bachelor's degree and 4–6 years of related experience.
• Bachelor's degree in Nursing preferred
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required or NP - Nurse Practitioner - Current State's Nurse Licensure
• For Superior: Resource Utilization Group (RUG) certification must be obtained within 90 days of hire required

Responsibilities

• Evaluates complex/high-risk members and recommends optimal care plans.
• Develops and assesses long-term care plans and coordinates with care management team and providers.
• Coordinates between member/family and care team to ensure appropriate person-centered care.
• Monitors and revises care plans to meet member goals and needs.
• Reviews member data to improve care quality and compliance with regulations.
• Provides education to members and families on disease processes and care options.
