Job summary

Centene is hiring for a hybrid RN Care Coordinator role serving Hidalgo County, TX, with home visits and remote work. The position focuses on coordinating care for complex/high-acuity adult members (21+), developing personalized care plans, and educating members and families on available services and benefits.

The role requires RN licensure and 4–6 years of related experience, with RUG certification within 90 days of hire. Schedule is Monday–Friday, 8:00 AM–5:00 PM CST, with travel for visits.

Qualifications

• Graduate from an Accredited School of Nursing with 4–6 years of related experience.
• Bachelor of Nursing degree preferred.
• RN licensure in the state (or NP with current licensure).
• RUG certification must be obtained within 90 days of hire.

Responsibilities

• Evaluates needs of the most complex/high-risk members and recommends best outcomes.
• Develops/assesses long-term care/service plans with care management team.
• Coordinates between member/family and care providers for person-centered care.
• Monitors care/service plans and member status; revises plans as needed.
• Monitors complications and triggers potential entry into higher level of care.
• Reviews data to identify trends and improve quality of care.
• Reviews referrals and intake assessments to plan care.
• Collaborates with healthcare providers to adjust plans as needed.
• Collects and maintains all member information to ensure compliance.
• Educates members/families on disease processes, care gaps, and benefits.
• Acts as liaison between member, family, physician and facilities.
