Job summary

Centene Corp. is seeking an experienced RN care coordinator for a hybrid role serving Hidalgo County, TX and surrounding areas. The position combines home visits with remote work from home, Monday–Friday, 8:00 AM–5:00 PM CST, with no on-call duties.

The role requires 4–6 years in clinical nursing/care coordination for adult members, with RN or NP licensure. Travel is required to member visits and mileage is reimbursed.

Qualifications

• Requires 4–6 years of related clinical nursing and/or case management experience.
• RN or NP licensure with state licensure required.
• Bachelor of Nursing degree preferred.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome
• Develops and continuously assesses long-term care plans and collaborates with the care management team
• Coordinates between member/family and care provider team to ensure appropriate person-centered care
• Monitors care plans and member status, revising plans as needed to meet goals
• Monitors for complications and potential entry into higher levels of care or waiver eligibility
• Reviews referrals and intake assessments to develop appropriate care plans
• Collaborates with healthcare providers to facilitate member services and revise care plans as needed
• Collects and maintains member information to ensure compliance with guidelines
• Provides education to members and caregivers on disease processes and services
• Acts as liaison between member/family, physician, and facilities/agencies
• Educates on community resources and coordinates service authorization
• May perform home visits and travel for member visits
• Partner with leadership to improve quality of care and service delivery
• May precept clinical new hires and guide onboarding journey
• Engages with teams to measure readiness and performance
• Performs other duties and travels as required
• Complies with policies and standards
