Centene Corporation is seeking a Hybrid RN Care Manager to support Duals AIP adult members with complex health needs. This role involves home visits, scheduling from a home office, and coordinating LTSS needs across Hidalgo County, TX.

The position emphasizes clinical leadership, care planning, and collaboration with providers to deliver person-centered care while adhering to state and federal guidelines.

Qualifications

• Graduate from an Accredited School of Nursing.
• BSN degree preferred.
• 4–6 years of related clinical nursing experience.

Responsabilités

• Assess and coordinate care for complex/high acuity members.
• Develop and monitor long-term care/service plans.
• Coordinate with care teams and providers for needed services.
• Educate members and families on care options and benefits.
• Maintain documentation and ensure compliance with guidelines.
