Centene Corporation in the Dallas area is seeking a hybrid RN Care Manager to assess and coordinate medical and supporting services for complex high-acuity populations. You will develop personalized care plans, conduct home health assessments, and coordinate LTSS needs while visiting members from a home office.

Schedule is Mon-Fri, 8:00 AM–5:00 PM CST with mileage reimbursement for visits. The role requires an RN license with 4–6 years of clinical nursing experience.

Qualifikationen

• RN licensure required in state or compact state
• BSN preferred but not required
• 4–6 years of related clinical nursing experience
• Strong clinical documentation and communication skills

Aufgaben

• Evaluate service needs of high risk/high acuity members and propose care plans
• Develop and assess long-term care plans and coordinate with providers
• Coordinate between member/family and care providers for person-centered care
• Monitor progress toward care plan goals and update as needed
• Document and maintain all member information and care management activities
• Educate members and families on disease processes and care options
• Act as liaison between member/family, physician, and facilities/agencies
