Job summary

Centene is seeking a Hybrid RN Care Manager to coordinate care for dual-eligible adults with complex medical needs in the South Dallas area. You will assess needs, develop personalized care plans, and connect members and families with appropriate services and benefits.

The role involves home visits, scheduling flexibility from a home office, and collaboration with care teams to ensure person-centered care while meeting regulatory requirements.

Qualifications

• RN with nursing degree and active license.
• 4–6 years of related clinical nursing experience.
• BSN preferred but not required.

Responsibilities

• Evaluate needs of complex members and propose care plans.
• Develop and monitor long-term care/service plans.
• Coordinate between member/family and care providers.
• Monitor and adjust care plans as needed to meet goals.
• Educate members and families on services and benefits.
• Liaise with healthcare providers to facilitate services.
