Centene Corporation in Dallas. An experienced RN Care Manager is needed to assess and coordinate medical and support services for complex members across the continuum of care. This hybrid role requires performing home visits and developing personalized care plans to ensure high-quality, cost-effective care.

Schedule: Monday–Friday, 8:00 AM–5:00 PM CST. Requires RN licensure and 4–6 years of clinical nursing experience; BSN preferred. Travel to service areas is expected with mileage reimbursement.

Qualificações

• RN licensure in good standing with state requirements.
• 4–6 years of related clinical nursing or care coordination experience.
• Bachelors degree in nursing preferred; strong documentation skills.
• Ability to perform home visits and coordinate LTSS services.
• Excellent communication and problem-solving abilities.

Responsabilidades

• Assess member needs and develop personalized care/service plans.
• Coordinate between members, families, and care teams for services.
• Monitor and revise care plans to meet goals and changes in condition.
• Conduct home visits and educate members on services and benefits.
• Ensure compliance with state/federal guidelines and documentation.
