Humana Inc. is seeking a Manager, Care Management to lead a team of care managers supporting members with transitional needs, chronic conditions, and barriers impacting health, independence, and well‑being.

This remote role prioritizes telephonic, member‑centered care management and alignment with Humana’s broader care objectives. The role requires coaching and developing associates, monitoring performance, and using data to identify trends and drive continuous improvement.

Qualifications

• Active, unrestricted RN license with no disciplinary action.
• 5+ years of case management experience.
• 2 years of leadership experience.
• Strong written and oral communication skills.
• Ability to analyze performance data and identify trends.

Responsibilities

• Provide day-to-day leadership, direction, coaching, and development for care management associates.
• Monitor performance and deliver timely feedback aligned to processes.
• Manage performance against quality, productivity, compliance, and documentation standards.
• Conduct regular meetings and reviews to support associate growth.
• Promote an engaged team environment and cross-functional collaboration.
• Use data to identify risks and drive workflow improvements.
• Ensure audit readiness and adherence to policies and privacy requirements.
• Support onboarding, training, and ongoing education for staff.
