Qualifications

• Active, unrestricted RN license with no disciplinary action.
• 5+ years of case management experience.
• 2+ years in direct or indirect leadership roles.
• Ability to analyze performance data and identify trends.

Responsibilities

• Provide day-to-day leadership, direction, coaching, and development for care management associates.
• Monitor performance, deliver feedback, and document conversations per HR policy.
• Manage performance against quality, productivity, compliance, and documentation metrics.
• Conduct regular one-on-one and team meetings, reviews, and development discussions.
• Promote an engaged, inclusive team environment with clear communication and recognition.
• Escalate complex issues requiring manager review or intervention.
• Support associates through process changes, system enhancements, and shifts in priorities.
• Analyze trends to identify risks and opportunities for improvement.
• Use data and insights to recommend actions improving workflows and outcomes.
• Communicate team needs and performance to stakeholders across the organization.
