Job summary

Humana Inc. is seeking a Manager, Care Management to lead a team of care managers supporting members with transitional needs and chronic conditions. This remote role drives telephonic, member-centered care and aligns with Humana’s care management priorities.

The role focuses on talent development, accountability, and data-driven improvements to member outcomes, ensuring compliance and high-quality care across the team. Travel may be required occasionally.

Qualifications

• Active, unrestricted RN license with no disciplinary action.
• 5+ years of case management experience.
• 2+ years of direct or indirect leadership experience.
• Ability to articulate ideas effectively in written and oral forms.
• Ability to analyze performance data, identify trends and risks, and implement actions.
• Experience supporting change management and process improvements with flexibility.

Responsibilities

• Provide day-to-day leadership, direction, coaching, and development for care management associates.
• Monitor performance and document conversations per HR processes.
• Manage performance against quality, productivity, compliance, and documentation standards.
• Lead regular team and individual meetings to support performance and growth.
• Promote an engaged, inclusive team environment through clear communication and recognition.
• Escalate complex member or process issues for manager intervention.
• Support onboarding, training, and ongoing education for telephonic care management staff.
