Job summary

Humana Inc. is seeking a Manager of Care Management to lead a team of care managers supporting members with transitional needs, chronic conditions, and barriers to health and independence.

This remote role focuses on telephonic, member-centered care management with alignment to Humana’s care management objectives. The role requires an active RN license, strong leadership, and a track record in improving quality and efficiency.

Qualifications

• Active, unrestricted RN license with no disciplinary action.
• 5+ years of case management experience.
• 2 years of direct or indirect leadership experience.
• Ability to analyze performance, productivity, quality, audit, documentation, and operational data.

Responsibilities

• Provide day-to-day leadership, direction, coaching, and development for assigned care management associates.
• Establish clear expectations, monitor performance, deliver feedback, and document conversations per HR processes.
• Manage associate performance against quality, productivity, compliance, and member outcome expectations.
• Conduct regular one-on-one and team meetings, call reviews, and development discussions.
• Promote an engaged, inclusive team environment through communication, recognition, collaboration, and development.
• Serve as escalation resource for complex member or process-related issues.
• Support associates through process changes, system enhancements, and shifting business priorities.
• Review performance trends to identify risks and opportunities for improvement.
• Use data to recommend actions that improve workflows and outcomes.
