Humana is seeking a Manager of Care Management to lead a telephonic, member‑centered team focused on transitional needs and chronic conditions. You will coach associates, monitor performance, and drive improvements using data insights while aligning to Humana’s care management objectives.

The role emphasizes developing talent, ensuring compliance, and maintaining high member outcomes in a fully remote setting. Strong RN background and 5+ years in case management are required.

Qualifications

• Active, unrestricted RN license.
• 5+ years of case management experience.
• 2 years of direct or indirect leadership experience.
• Demonstrated written and oral communication skills.
• Ability to analyze performance, productivity, and compliance data; identify trends and risks.

Responsibilities

• Provide day-to-day leadership, direction, coaching, and development for assigned care management associates.
• Monitor performance and deliver timely feedback; document performance-related conversations.
• Manage performance against quality, productivity, compliance, documentation, attendance, and member outcomes.
• Conduct regular meetings, call reviews, and development discussions to support performance.
• Promote an engaged team environment and effective cross‑functional collaboration.
• Escalate complex member or process issues requiring leadership review.
• Support process changes, system enhancements, and shifting business priorities.
• Identify risks and opportunities from data trends and take corrective actions.
• Communicate team needs and performance to stakeholders and leaders.
• Ensure audit readiness by adhering to policies and privacy requirements.
