Job summary

The position is remote with travel across Illinois, requires a valid RN/LCSW/LCPC license and 5+ years in health care/care management, plus 2+ years of leadership.

Qualifications

• Active IL RN, LCSW or LCPC license required.
• 5+ years healthcare or care management experience.
• 2+ years of leadership experience.
• Ability to travel across Illinois.
• Proficient with electronic medical records and data trends.

Responsibilities

• Lead cross-functional care management teams for coordination of care.
• Identify members for case or disease management activities.
• Monitor case management and discharge planning processes.
• Develop system-view reports and identify actions to control trends.
• Create audit plans to ensure compliance with state contracts and metrics.
• Collaborate with leadership to develop reporting tools for clinical performance.
• Hire, mentor, coach, and manage performance of the care coordination team.
• Onboard new associates including HR tasks and setup of software/hardware.
• Monitor staff performance against utilization and care coordination benchmarks.
