Job summary

PACS is seeking a licensed Physical Therapist to evaluate patients within 24 hours of referral, develop treatment plans, and supervise PT assistants in a rehabilitation setting.

The role requires a Bachelor’s in PT, proficiency with Casamba and Point Click Care, and strong communication for care planning and discharge processes. You will document progress and participate in in-services and conferences as needed.

Qualifications

• Education: Bachelor’s degree in Physical Therapy; Masters/ Doctorate preferred.
• Proficient with Casamba and Point Click Care.
• Ability to read technical procedures and policy manuals.
• Mathematical skills: fractions, percentages, ratios, proportions.
• Strong reasoning: solve problems and interpret instructions.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop treatment plans and obtain approval from referring physician.
• Treat patients per the physician plan and document progress.
• Supervise PT assistants and staff in direct patient care.
• Record evaluations, daily and weekly progress notes, and discharge documentation.
• Instruct families and staff in maintenance and caregiver training for discharge.
