Job summary

PACS is seeking a licensed Physical Therapist to evaluate patients within 24 hours of referral, develop and obtain approval for an individualized treatment plan, and supervise therapy staff in direct patient care.

You will participate in discharge planning, document daily progress, and collaborate with the care team to promote mobility, safety, and quality outcomes. Proficiency with Casamba and Point Click Care is preferred.

Qualifications

• Bachelor’s degree in Physical Therapy; Masters/Doctorate preferred.
• Licensed in the state and maintain CE requirements.
• Ability to read technical procedures and policies.

Responsibilities

• Evaluate patients within 24 hours of referral and develop treatment plans.
• Supervise PT assistants and students in direct patient care.
• Document evaluations, daily progress, and discharge plans.
• Participate in care conferences and staff training.
• Coordinate durable medical equipment for mobility.
