Job summary

PACS is seeking a licensed Physical Therapist to evaluate patients within 24 hours of referral and develop approved treatment plans. You will supervise PT staff, coordinate with physicians and nursing teams, and contribute to discharge planning for optimal patient mobility.

Qualifications

• Licensed Physical Therapist in the state.
• Ability to supervise PT assistants, aides and students.
• Experience with Casamba and Point Click Care software.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for a treatment plan from referring physician.
• Treat patients per the physician plan and document progress.
• Supervise PT assistants, aides and students in patient care.
• Record evaluations, daily/treatment notes, progress notes and recertifications.
• Participate in discharge planning and caregiver training for home care.
