Job summary

PACS is seeking a licensed Physical Therapist to evaluate patients within 24 hours of referral and develop effective treatment plans. You will supervise PT assistants, document progress, and participate in discharge planning and caregiver training. Strong computer skills with Casamba and Point Click Care are required.

Your role includes collaboration with the health team, participation in meetings, and adherence to state practice acts and payer requirements.

Qualifications

• Bachelor’s degree in Physical Therapy required and active state license.
• Proficient in computer data entry in Casamba and Point Click Care.
• Ability to read and implement facility policies and procedures.

Responsibilities

• Evaluate patients within 24 hours of referral and develop a treatment plan.
• Supervise PT assistants and training for staff.
• Record evaluations and daily/weekly progress notes per policy and state regulations.
• Assist with discharge planning and caregiver training; order durable medical equipment as needed.
• Participate in meetings and in-service trainings; communicate patient progress with supervisor.
