Qualifications

Responsibilities

• Coordinate and monitor care gaps to achieve PCP goals.
• Provide CPSS supervision and ensure CPSS training within 90 days.
• Advise on recovery-focused planning with beneficiaries.
• Maintain documentation and quality assurance for service notes.
• Collaborate with program staff and community resources.
• Manage team caseload sizing and staff supervision.
