Job summary

START Wellness is seeking a Peer Support Specialist Program Supervisor to enhance integrated care for members. You will plan, coordinate, and monitor interventions to close gaps in care and uphold person-centered plans in a dynamic behavioral health setting.

Responsibilities include supervision, training, and ensuring CPSS practices align with recovery principles while coordinating with the care team and community resources. Location in NC, with driving as needed.

Qualifications

• Bachelor’s degree in health, psychology, sociology, social work, nursing, or related field or RN licensure.

Responsibilities

• Promote whole person, integrated care by planning, coordinating and tracking care.
• Monitor care gaps to achieve member PCP goals.
• Coordinate care transitions and post-discharge follow-up within 24 hours.
