Qualifications

• Qualified Professional per NC definitions as required.
• Bachelor’s degree in health, psychology, sociology, social work, nursing, or licensure as RN.
• Two years of direct experience with members with behavioral health conditions or I/DD/TBI populations.
• CPSS certification and supervisory training within specified timelines.

Responsibilities

• Promote whole-person, integrated care through care coordination.
• Plan, coordinate, track, and close gaps in care for members.
• Provide supervisory oversight to program staff.
• Facilitate recovery-oriented, person-centered plans.
• Coordinate care transition and crisis response as needed.
