Job summary

BoldAge PACE in California is seeking a Medical Director to deliver high-quality primary and continuous healthcare to PACE participants. You will mentor the IDT, provide clinical guidance, and ensure adherence to standards across care transitions.

The role emphasizes leadership in clinical performance, coordination with the Chief Clinical Officer, and ongoing quality improvement within a patient-centered program for seniors at home and in clinics.

Qualifications

• Master's or doctoral degree in nursing required.
• Current NP/PA license with prescriptive authority.
• State license and federal DEA registration required.
• National board certification as NP/PA; geriatrics/palliative preferred.
• Geriatrics, palliative care or complex chronic care training preferred.
• BLS certification required or within 90 days.
• 5+ years advanced practice clinical experience; 2+ years with medically complex older adults.
• Experience leading clinical teams, mentoring providers, or improving workflow.

Responsibilities

• Provide high-quality, person-centered primary care to an assigned panel of PACE participants.
• Serve as clinical resource for primary care providers and the IDT; provide real-time guidance.
• Lead IDT meetings with clear recommendations and actionable care plans.
• Collaborate with IDT to ensure appropriate referrals and continuity of care.
• Monitor participant medications, reconcile, deprescribe when appropriate.
• Support clinic coordination, provider workflow, and adherence to standards.
• Participate in 24/7 on-call rotation and ensure proper handoffs.
• Contribute to discharge planning and transitional-care processes.
• Partner with leaders on quality improvement and utilization management.

Education

Master’s or doctoral degree in nursing

Current NP/PA license with prescriptive authority

State license and federal DEA registration

National board certification as NP/PA

Geriatrics, palliative care or complex care training

BLS certification

5+ years advanced practice clinical experience (2+ years geriatrics)

Clinical leadership, mentoring, or quality improvement experience
