Veracity Ventures Inc. in Elk Grove, CA is seeking a Medical Director I to provide comprehensive primary medical care for PACE participants, collaborating with the Interdisciplinary Team to develop individualized care plans that promote independence and quality of life.

This physician will oversee participant care across hospitals and long-term care settings while mentoring Nurse Practitioners and supporting IDT members in a value-based, preventive medicine environment.

Qualifications

• MD or DO with an ACGME-accredited residency.
• Active California medical license without restrictions.
• Active DEA registration.
• Active California CDS certification.
• Eligible for Medicare and Medi-Cal provider enrollment.
• Willingness to participate in rotating night and weekend call.

Responsibilities

• Perform comprehensive medical assessments for PACE participants.
• Develop individualized care plans with the Interdisciplinary Team (IDT).
• Provide ongoing primary medical care and clinical oversight.
• Coordinate participant admissions and discharges from hospitals and long-term care settings.
• Conduct routine visits to participants residing in nursing facilities.
• Collaborate with community physicians and specialists to ensure coordinated care.
• Participate in daily Interdisciplinary Team meetings.
• Guide clinical decision-making for complex geriatric patients.
• Supervise and support Nurse Practitioners and other IDT members.
• Coordinate specialty referrals and follow-up care.
• Maintain compliance with Medicare, Medi-Cal, and PACE requirements.
• Promote participant-centered, value-based healthcare delivery.
• Foster long-term relationships with participants and caregivers.
