BoldAge PACE is seeking a Medical Director in California to deliver high-quality primary and continuous healthcare to PACE participants. You will mentor the interdisciplinary team, provide clinical guidance, ensure timely documentation, and promote patient-centered care across transitions.

The role requires an active NP/PA license or equivalent, board certification in geriatrics or related fields, and at least five years of advanced practice experience in geriatrics or home-based care.

Qualifications

• Master’s or doctoral degree in nursing or related field.
• Current, unrestricted NP/PA license and prescriptive authority.
• Board certification in geriatrics, palliative care, or complex chronic care preferred.
• BLS certification required or to be obtained within 90 days.
• Experience leading clinical teams and mentoring providers.

Responsabilités

• Provide high-quality primary care to an assigned panel of PACE participants.
• Serve as a clinical resource for primary care providers and the IDT.
• Coordinate day-to-day clinical operations and provider workflow.
• Lead IDT meetings with clear, actionable clinical recommendations.
• Coordinate referrals, medication management, and transitions of care.
• Contribute to quality improvement, utilization management, and policy initiatives.
• Participate in 24/7 on-call rotation and ensure effective handoffs.

Formation

Master’s or doctoral degree in nursing

Nurse Practitioner/Physician Assistant license

Board certification in geriatrics, palliative care, or complex chronic care

BLS certification

Strong clinical leadership and mentoring experience
