Job summary

EveryAge PACE seeks a Medical Director to provide executive clinical leadership for PACE programs at Carolina SeniorCare and Elderhaus in North Carolina. The role blends strategic direction with direct patient care as a PCP within PACE, guiding outcomes, compliance, and evidence-based policies.

You will lead quality initiatives, mentor physicians, and collaborate with hospitals, specialists, and community partners to advance population health in a dynamic, interdisciplinary setting.

Qualifications

• MD or DO from an accredited medical school.
• Board-certified in Internal Medicine or Family Medicine.
• DEA Registration and NC Controlled Substance Certification.
• Current BLS/CPR certification.
• Minimum 2 years caring for frail/elderly populations.
• Leadership or medical director experience preferred.

Responsibilities

• Provide overall clinical leadership and medical oversight for EveryAge PACE participants.
• Lead initiatives to improve clinical outcomes, quality and participant experience.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Support transitions of care, hospital utilization management and medication coordination.
• Lead quality improvement activities and ensure regulatory compliance.
• Represent EveryAge PACE in professional organizations and partnerships.
