SNF Staffing Solutions seeks an experienced MDS Coordinator to manage MDS 3.0 processes and care planning for residents across several New Mexico facilities. The role requires RN licensure and at least two years in a skilled nursing setting.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Federal PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and the Minimum Data Set.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure MDS/documentation aligns with state and federal regulations.
• Monitor and document MDS status and data entry in appropriate systems.
• Prepare scheduling and notices for care planning conferences.
• Assist with communication of outcomes to staff, residents, and families.
