The ideal candidate has a Bachelor's in social work or related field and 2+ years delivering services to elderly populations, with strong communication and organizational skills.

Qualifications

• Bachelor's degree in social work or related field preferred.
• Two+ years delivering social services to elderly and/or family populations preferred.
• Working knowledge of supportive services and local community resources.

Responsibilities

• Advocate for residents and organize programs to meet their needs.
• Coordinate activities and services for residents.
• Liaise with community agencies, network with service providers, and negotiate discounts where possible.
• Engage residents to identify needs and refer to community resources.
• Develop a resource directory listing state/local providers.
• Sponsor educational events on health care, services, life skills and referrals.
• Provide quality customer service and address resident concerns.
• Assist residents in obtaining services like meals, transportation, and personal services.
• Monitor feedback from residents and families and relay it to staff.
• Manage relationships internally and externally with strong communication skills.
