Job summary

National Church Residences is hiring a Service Coordinator for Mabess Manor in Campbellsport, Wisconsin. This part-time position offers an 8am-5pm schedule with weekdays only and a pay rate of $24.70 per hour for 20 hours per week.

As a Service Coordinator, you will advocate for residents, coordinate programs, and liaise with community providers to connect residents with essential services. Bachelor’s degree in social work or related field is preferred, with 2+ years of relevant experience

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Two or more years delivering social services to elderly or family populations preferred.
• Working knowledge of supportive services and local community resources.

Responsibilities

• Advocate for residents and organize services to meet needs.
• Coordinate programs and activities for residents.
• Serve as liaison to community agencies and negotiate discounts where possible.
• Develop a Resource Directory of local service providers.
• Sponsor educational events on health care, social services, life skills and referrals.
• Provide quality customer service and assist residents with basic needs.
• Assist residents in accessing community services like transportation, meals, and day care counseling.
• Document resident feedback and relay concerns to appropriate parties.
• Maintain professional relationships with internal teams and external partners.
