National Church Residences in Stamford, CT is seeking a part-time Service Coordinator to support senior residents. You will coordinate programs, advocate for residents, and connect them with community resources to improve daily living.

This role focuses on engaging residents, developing service referrals, and maintaining a resource directory, while partnering with local agencies. A Bachelor's in social work and strong organizational skills are preferred.

Formación

• Bachelor's degree in social work or related field preferred.
• Two or more years of experience delivering social services to elderly and/or family populations preferred.
• Working knowledge of supportive services and local community resources.
• Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsabilidades

• Advocate, organize, problem-solve, and deliver results for residents.
• Coordinate programs and activities for residents.
• Serve as liaison to community agencies, network with service providers, and seek low-cost services or discounts.
• Engage residents to identify needs and make referrals to services.
• Develop a Resource Directory with listing of providers.
• Sponsor educational events on health care, agency support, life skills and referrals.
• Provide quality customer service to residents, addressing concerns and helping with basic needs.
• Assist residents in obtaining and utilizing community services such as meals, transportation, housekeeping, and financial assistance.
• Monitor, document, and relay feedback from residents and families.
• Manage internal and external relationships with strong communication.
