Job summary

PACS is seeking an Activity Assistant to support the Activity Director in delivering a creative and interactive activities program for residents in a post-acute care setting.

You will help plan and conduct activities, coordinate transportation, maintain calendars and attendance, and assist with assessments and care plans, while ensuring compliance with standards and resident engagement.

Qualifications

• High school diploma or equivalent required.
• Preferable one-year experience in a long term care facility.

Responsibilities

• Assist in planning and conducting individual, small and large group activities for residents.
• Assist in developing monthly activity calendars and maintaining attendance records.
• Support communication between staff, residents, families, and external agencies to meet residents' needs.
• Assist with discharge planning and development of activity care plans and resident assessments.
• Arrange transportation for residents to outings, appointments, or discharge as needed.
• Keep the Activity Department clean, organized, and secure.
