Job summary

PACS is seeking an Activity Assistant to support the Activity Director in creating a creative and interactive activities program for residents in a post-acute care setting. You will help plan, coordinate, and conduct activities that meet residents' physical, mental, and psychosocial needs, with attention to documentation and safety.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems and interpret instructions in various formats.

Responsibilities

• Participate in planning and conducting individual, small and large group activities.
• Assist in communication between employees, residents, families and public to meet resident needs.
• Assist in development of monthly activity calendar and attendance records.
• Assist in discharge planning and activity care plans.
• Arrange transportation for residents when necessary for outings, appointments, or discharge.
• Encourage residents to participate in self-initiated activities and provide materials as needed.
• Keep the Activity Department clean, orderly and secured.
