You will plan monthly calendars, coordinate group and individual activities, arrange transportation, and assist with assessments and care plans.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.

Responsibilities

• Plan and conduct individual, small, and group activities.
• Assist in communicating between staff, residents, families, and agencies.
• Develop monthly activity calendars and maintain attendance records.
• Assist in discharge planning and activity care plans; arrange transportation when needed.
