Job summary

PACS is seeking an Activity Assistant to support the Activity Director in delivering a creative and interactive activities program that meets the residents' physical, mental, and psychosocial needs.

You will plan and conduct activities, coordinate transportation, maintain attendance records and calendars, and help with discharge planning and activity care plans while ensuring safety and quality.

Qualifications

• High school diploma or equivalent is required.
• One year of experience in a long term care facility is preferred.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in communication between staff, residents, families and agencies.
• Develop and maintain monthly activity calendars and attendance records.
• Assist with discharge planning and activity care plans.
• Coordinate transportation for residents to outings and appointments.
• Keep the Activity Department clean, orderly and secure.
