Volunteers of America Colorado is seeking a Service Coordinator for the Montrose Affordable Housing Portfolio. You will conduct needs-based assessments, coordinate housing-related services, and connect residents 62+ with income resources to support independent living.

The role focuses on Trauma Informed Care, Harm Reduction, and Motivational Interviewing, working with a Property Manager, and building strong community partnerships to expand resident resources.

Qualifications

• Bachelor’s degree in human services, social work, or a closely related field.
• Minimum of one year of experience with seniors and/or people with disabilities.
• Willing to obtain First Aid/CPR and keep current.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal and organizational skills.
• Proficient with Windows and database usage.

Responsabilités

• Ensure residents' voices are heard and actively participate in programming.
• Facilitate referrals from the Property Manager, family, service providers, and residents.
• Assess resident needs with surveys, visits, and group meetings; document findings.
• Coordinate services via HUD-aligned modalities to support independence.
• Provide information on services and connect residents to multiple options.
• Encourage residents to arrange required services to maintain independence.
• Monitor provider activities to ensure quality and cost efficiency.
• Maintain an up-to-date community resource guide and collaborations.
