Job summary

VOA Colorado is seeking a Service Coordinator for the Montrose affordable housing portfolio. The role focuses on needs-based assessments, connecting residents to health, wellness and income resources, and coordinating services to help residents age in place.

You'll collaborate with the Property Manager, maintain records, develop resident programs, and build partnerships with local service providers to expand opportunities for older adults and neighbors with disabilities.

Qualifications

• Bachelor’s degree in human services, social work, or a related field.
• At minimum, one year of experience with senior citizens and/or people with disabilities.
• Willing to obtain first aid, AED and CPR certification and keep current.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal and organizational skills.
• Proficient in Windows environment and working with databases.

Responsibilities

• Assess resident needs via surveys, visits, and meetings; identify services.
• Coordinate and monitor provider services to ensure quality and cost effectiveness.
• Provide information on services and connect residents to options.
• Develop and facilitate resident support groups and workshops.
• Maintain up-to-date resource guides and resident records.
• Relate with care providers, agencies, and multidisciplinary teams to support independence.
