Job summary

VOAC is seeking a Service Coordinator to manage housing-related services for residents in an affordable housing portfolio. The role emphasizes trauma-informed care, harm reduction, and motivational interviewing to promote independence and aging in place.

The position requires coordinating health, wellness, and educational programs, documenting client interactions, and maintaining strong community partnerships. Full-time hours with travel within Colorado locations are typical.

Qualifications

• Bachelor’s degree in human services, social work, or related field.
• At least one year of experience with senior citizens and/or people with disabilities.
• Willing to obtain first aid, AED and CPR certifications and keep current.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal skills and good organizational skills.
• Proficient in using Windows environment and working with databases.
• Bilingual in a language represented in the resident community is a plus.

Responsibilities

• Assess services needed by residents through surveys, visitations, and group meetings.
• Coordinate and monitor services and activities of providers to residents to ensure quality of care and cost effectiveness.
• Provide residents information on specific services and program providers with several options.
• Develop and facilitate groups and workshops to respond to resident needs.
• Maintain up-to-date records on all residents and service providers in the community.
