Job summary

TEEMA seeks an organized Home Care Scheduler to coordinate scheduling for Home Care Assistants and field staff. You will work under the Home Care Supervisor to ensure timely, high-quality home care, manage schedule changes, and verify care logs in EMR systems.

The role requires at least 1 year of scheduling experience in home health or home care, strong communication, and the ability to multitask in a fast-paced environment. On-site in San Jose, CA, with a Monday–Friday schedule.

Qualifications

• High School Diploma or GED equivalent.
• Minimum of one year of experience as a scheduler within a home health or home care agency setting.
• Experience with EMR scheduling software is preferred.
• Ability to work with diverse populations and support vulnerable seniors.

Responsibilities

• Coordinate scheduling for home care assignments under the Home Care Supervisor.
• Ensure EMR authorizations are in place for approved services.
• Collect Care Logs and Timesheets from vendors and in-home staff.
• Provide reminders and instructions to ensure a smooth care experience.
• Maintain accurate records of all scheduled assignments and visit outcomes.
• Complete required documentation in a timely and accurate manner.
• Protect privacy and confidentiality of patient data and company procedures.
• Follow policies and participate in training and team meetings.
• Communicate workload and priorities with Supervisors and coordinators.
• Be flexible for varied shifts including evenings, weekends, and overtime if needed.
