Job summary

TEEMA seeks an organized Home Care Scheduler to coordinate scheduling for Home Care Assistants and field staff. You will work under the Home Care Supervisor to ensure timely, high-quality home care, manage schedule changes, and verify care logs in EMR systems.

The role requires at least 1 year of scheduling experience in home health or home care, strong communication, and the ability to multitask in a fast-paced environment. On-site in San Jose, CA, with a Monday–Friday schedule.

Qualifications

• High School Diploma or GED equivalent.
• Minimum of one year of experience as a scheduler within a home health or home care agency setting.
• Experience with EMR scheduling software is preferred.

Responsibilities

• Coordinate scheduling for home care assignments under the Home Care Supervisor.

In this key administrative role, you will coordinate, track, and optimize scheduling assignments for Home Care Assistants and field staff. Working under the guidance of the Home Care Supervisor, you will ensure our patients receive timely, high-quality care at home while managing schedule adjustments, processing EMR authorizations, and verifying care logs. Serving as a crucial liaison between patients, families, field staff, and vendors, you will deliver exceptional customer service and keep daily operations running seamlessly. Duties & Responsibilities

Effectively coordinate the scheduling of home care personal care assignments as directed by the Home Care Supervisor, including adjusting schedules based on changes and communicating promptly with all stakeholders (patients, families, home services staff, etc.).

Ensure required authorizations are in place in the Electronic Medical Record (EMR) for approved personal care services.

Ensure all Care Logs and Timesheets are received from vendors and in-home staff visits in a timely manner.

Consistently implement procedures to ensure a successful experience for providers, staff, and patients, such as making reminder calls, offering instructions, or sending materials ahead of time.

Maintain current and accurate records of all scheduled home care personal care assignments, including scheduled times and visit outcomes (e.g., visit occurred or did not occur, reasons for rescheduling).

Complete all required documentation in a timely and accurate manner.

Protect privacy and maintain strict confidentiality of all company procedures, patient data, and family information.

Follow company policies and procedures and participate in Quality Improvement activities, staff training, and team meetings.

Communicate regularly with the Supervisor, Home Care Coordinators, and Home Services staff regarding workload and priorities.

Complete all mandated trainings, education, and occupational health screenings in a timely manner.

Exercise flexibility in performing assignments as business needs evolve, including the willingness and ability to work a varied schedule that may include evenings, weekends, and overtime if needed.

Perform other related duties as assigned.

Education & Experience

High School Diploma or GED equivalent.

Minimum of one (1) year of experience as a scheduler within a home health or home care agency setting.
