Job summary

TEEMA Solutions Group is seeking a Home Care Scheduler in San Jose, CA to coordinate scheduling for Home Care Assistants and field staff. You will work with the Home Care Supervisor to ensure timely, high-quality in-home care, process EMR authorizations, and verify care logs while maintaining confidentiality.

Responsibilities

• Coordinate scheduling of home care personal care assignments as directed by the Home Care Supervisor.
• Ensure EMR authorizations are in place for approved services.
• Process and review care logs and timesheets from vendors and in-home staff.
• Announce and implement scheduling changes and communicate with patients, families, and staff.
• Maintain accurate records of all scheduled assignments and visit outcomes.
• Complete required documentation timely and accurately.
