Job summary

TEEMA Solutions Group is seeking a Home Care Scheduler to coordinate scheduling for Home Care Assistants and field staff in an on-site San Jose, CA setting. The role supports the Home Care Supervisor by ensuring timely, high-quality at-home care and by managing schedule changes and required EMR authorizations.

Responsibilities include communicating with patients, families, staff, and vendors, maintaining accurate records, and ensuring documentation and timesheets are completed promptly.

Qualifications

• High School Diploma or GED is required.
• Minimum of one year of experience as a scheduler within a home health or home care agency setting.
• Excellent organizational, communication, and interpersonal skills; ability to manage staff schedules in a fast-paced environment.

Responsibilities

• Coordinate scheduling of home care personal care assignments as directed by the Home Care Supervisor.
• Adjust schedules based on changes and communicate with patients, families, staff, and vendors.
• Ensure EMR authorizations are in place for approved services.
• Collect and verify care logs and timesheets from vendors and staff.
• Maintain accurate records of all scheduled assignments and visit outcomes.
• Complete required documentation timely and accurately and protect patient privacy.
• Communicate workload and priorities with Supervisor and Home Care Coordinators regularly.
• Attend trainings and participate in quality improvement activities.
