Ambercare – an Addus family company in Socorro, NM is seeking a Full Time Service Coordinator to schedule and supervise in-home care workers and clients. You will conduct in-home visits to monitor care plans and ensure compliance with regulations, supporting older adults and individuals with disabilities to live safely at home.

This role requires coordinating schedules, ensuring coverage, and maintaining high-quality service in a tight-knit team environment.

Qualifications

• Minimum of 2 years’ experience in senior services or related social service field.
• Excellent oral and written communication skills.
• Strong interpersonal skills.
• Ability to prioritize and handle high call volumes.
• Client-focused and customer service-driven.
• Proficient in Microsoft Word, Excel, and Outlook.
• Must have reliable transportation, valid driver’s license and insurance for travel within the branch service area

Responsibilities

• Creates work schedules by entering schedules into the system and adjusting for changes.
• Provides alternate coverage to ensure care plans are followed.
• Contacts care providers and clients to provide service updates.
• Monitors client condition and performance of Home Care Aides.
• Maintains compliance with Medicare, Medicaid, HIPAA and internal rules.
• Conducts home visits to welcome new clients and review Welcome Packet.
• Prepares evaluation reports and case notes as needed.
• Maintains timely completion of all assigned home visits and attends branch meetings.
