Job summary

Ambercare is seeking a Full Time Service Coordinator in Socorro, NM. You’ll schedule and supervise in-home care workers, conduct home visits, and monitor care plan adequacy to meet client needs.

Responsibilities include coordinating schedules, ensuring uninterrupted services, and maintaining HIPAA and quality standards. Ideal candidates have 2+ years in senior services and strong communication and organizational skills.

Qualifications

• Minimum of 2 years’ experience in senior services or related social service field.
• Excellent oral and written communication skills.
• Strong interpersonal skills.
• Ability to prioritize and handle high call volumes.
• Client-focused and customer service-driven.
• Proficient in Microsoft Word, Excel, and Outlook.
• Must have reliable transportation, valid driver’s license and insurance for travel within the branch service area.

Responsibilities

• Creates and manages schedules for in-home care workers and clients.
• Conducts in-home visits to monitor service quality and evaluate care plans.
• Maintains compliance with company, Department on Aging, Medicare, Medicaid and HIPAA standards.
• Contacts care providers and clients to provide service updates.
• Attends weekly branch meetings and records evaluations and notes.
