Competenze

• Active, unencumbered RN license or equivalent clinical license in the state of operations.
• Plus 2+ years clinical care experience in consumer/healthcare settings.
• Experience in wellness or managed care with clinical issues is preferred.

Mansioni

• Provide care coordination to members via telephonic and field-based care management.
• Educate members on health care programs and address health goals.
• Complete health screenings, assessments and care planning, including home visits if needed.
