Job summary

HCSC is seeking a care coordination professional to support members through telephonic and field-based care management. The role informs and educates on health programs, engages members in achieving health goals, handles member inquiries, and supports clinical operations with providers and community resources.

Responsibilities include health screenings, comprehensive assessments, and care planning, which may require home or facility visits for complex populations.

Qualifications

• RN, LCPC, LCSW or LMFT license with 2 years clinical care experience.
• Current, valid, unrestricted license in IL or reciprocity; obtain RN license if relocating.
• Plus 3 years wellness/managed care experience addressing clinical issues.
• Knowledge of health and employer trends in wellness marketplaces.
• Strong verbal and written communication with staff, providers and community resources.
• Proficient with Word, Excel, PowerPoint; databases and web apps.
• Valid driver's license with ability to travel within territory.

Responsibilities

• Provide care coordination to members via telephonic and field-based management.
• Inform and educate members on health care programs to address needs.
• Engage members in adherence to personal health goals and respond to inquiries.
• Support clinical operations with provider and member activities.
• Complete member health screenings, assessments and care planning; may visit homes or facilities.
