Job summary

CVS Health is seeking a Senior Epic Analyst focused on Healthy Planet and Care Coordination to design, build, and optimize Epic solutions that support population health management and care delivery. You will mentor junior analysts, partner with clinical leaders, and translate program strategy into scalable Epic configurations.

You will lead cross-functional design sessions, manage backlogs, and ensure accurate dashboards and data feeds for reporting.

Qualifications

Bachelor's degree or equivalent experience in a health/IT field.
4+ years Epic experience in Healthy Planet and/or care coordination modules.
Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules preferred.
Demonstrated ability designing workflows for population health or value-based care.
Strong transitions-of-care knowledge and cross-continuum care delivery understanding.

Responsibilities

Design, build, and optimize Epic Healthy Planet registries, care plans, care teams, and health maintenance topics.
Configure risk stratification tools, care gaps, and outreach workflows within Healthy Planet.
Lead testing and validation of Healthy Planet builds for registry logic and reporting outputs.
Design and maintain care coordination workflows supporting transitions of care and referrals.
Optimize Care Everywhere and interoperability workflows for timely data exchange.
